The CMS gave Avera Morningside Heights Care Center an overall 3 out of 5 rating, slightly below Minnesota’s average rating of 3.2 for nursing homes.
Prairie View Senior Living was the second largest nursing home in Lyon County for the quarter, offering 45 available beds.
Avera Morningside Heights Care Center had no fines or penalties on record in the first quarter of 2026.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
